htaccess correct file format for IP access etc...

I need proper .htaccess file format to:

Allow access to directory for specified IPs. Allow to
Plus allow User Name and Password (htpasswd) access to same directory.

htpasswd in root.

User with allowed IP must NOT see authentication page.

User without allowed IP MUST see authentication page.
