Date from multiple entries

Hi everyone
Wonder if you could help me

have a table with 5 field
startdate, enddate, dose, equipment, patient

Data is set out as such

startdate - enddate - Dose -- Equipment - Patient
01/01/2010 01/02/2011 15 iv 101A
01/02/2011 01/03/2011 15 iv 101A
01/03/2011 01/05/2011 15 static 101A
01/05/2011 01/09/2011 15 static 101A
01/09/2011 01/01/2012 15 UVA 101A
01/01/2012 01/11/2012 10 UVA 101A


What I would like to have is the following tables when dose get's changed, equipment changes often therefore a new entry
happens, but only need to see when a dose gets changed per patient

start1 End1 start2 end2
01/01/2010 01/01/2012 01/01/2012 01/11/2012
