Calls dropping

So I've been scratching my head on this one. For sometime now I've had a user at one of our buildings report calls are dropping. I did a list measurements on that trunk and wasn't showing any slips. I've tested the ds1 board and it shows everything passing. I've even called are phone provider and they've monitored the trunk and they say everything is good.
Is there anything else I need to check out?

"I have been experiencing the telephone disconnecting during conversations since early last week. I knew you guys were working on phone lines this week end and I thought I'd ride it out. It happened again this morning-I'm talking and the call is dropped and the banner goes out on the desk set.

Additionally, I'm sometimes don't get a dial tone when using my head set or I sometimes have the lift the receiver twice to answer a call with the headset button."

 
banner goes out on desk set - does this mean screens goes blank?
sounds like it is a physical/hardware issue to me, no? The set losing dialtone, I would start by replacing set and line cord along with checking the jack...
 
It's a brand new phone. I'll check the cord and the jack.
 
PhoneTek - What a coincidence - I'm experiencing a similar problem but on a wider scale. My system is a G3R.

For the last 2 days, my switchboard (7 operators in another building) has been complaining of dropped inbound calls. Sometimes it's so quick the Caller ID lights up for a second and then disappears before they have a chance to answer. I've also received reports of outbound calls dropping after a few minutes, and "dead air" in the middle of the calls.

We use 6408 and 16 stations, and our swichboard is 302C's. I couldn't find a damn thing on my ds1 signal's, and my ASA server spits out a report every hour, 24x7. No slips, busy errored seconds or the like for the last 2 days. A display events showed nothing. My trunks are all up. My PBX maintenance vendor did a similar check and gave me a clean bill of health.

I pushed this out to my telephony carrier, and they say they can't find a darn thing. I've sent them dozens of call examples that I've received from my switchboard, but they can't find anything and are pointing at my PBX. I told them to go check with their inter-carrier group and ask if other carriers are reporting problems.

I find it hard to believe that all 7 consoles would go bad at once, but I don't know at this point. Did you find anything in your investigation?
