Auto Note Maker

Does anyone have a lisp or otherwise that can automatically generate a note with a leader and box(sizes automatically to the text) and prompt you for the text.

Any help would be appreciated. I have searched the usual places and can't find anything.

I have selected in my dimension style to surround text with a box, but this does not seem to work for leaders.

Do you know how I can get it to work?
