I'm a little confused by your question...
If I understand you correctly, Microsoft's SCCM (System Configuration Console Manager) tool should allow you to do this. It's a powerful tool that can break your domain if not used carefully. We utilize it as a tool to send OS images to workstations...
I do not know of any freeware that will drill down to an IP address that is causing this issue, but I can tell you some hardware tricks you can use to track down the user.
First, a question... Are you using a managed switch?

I am using it as the control source in the form. It is returning the correct result... it just doesn't store it... If my user needs to view when the feedback was received, they get today's date, instead of the date that they actually received it.
