If you are using DS1 cards and T1/E1's for your inbound calls I would suggest doing a:
"list measurements ds1 log [board location]"
make sure you are not taking any errors on any of them.
If you are taking on slip errors this indicates a clocking issue. You can easily correct this yourself...

Is 641 a local area code?
Do a "display ars analysis 1641" and see what route pattern it is using.

I do what 1a22ip does, but be carefull using a bridged soft number.
I usually create the soft number as a digital set with up to 5 call appearances, and create a bridged appearance for each on all 9+ sets.
The down side is anyone that has the bridged appearance can pick up that line and hear...
I would trace out the failed port and re-punch it.
Try a list trace on the extension associated with the bad port, and dial it. See if this gives you any clue.
Swap a good member with the one that doesn't answer. See if the problem stays with the port on the Audix or if it follows the line.
