Unable to create new OC4J instance.

Recently I have been unable to create a new OC4J Instance from EM - Oracle 9ias 9.0.2 on Red Hat AS 2.1

I get the following error:
The operation failed.. Instance: ias1.ed-oracle.xxx.yyy.com Message: See
base exception for details. Base Exception:
java.lang.NoClassDefFoundError:nullSee base exception for details

Almost 2 weeks of fighting with Metalink have yielded no results, has anyone experience of this and know of a solution?
