Pages overlay previous page when printing

When printing more than three pages of progress notes, printer overlays next page of notes over previous page.

We have a patient that has 7 pages of progress notes. Preliminary testing shows following results:
Page 1 printed with no issue.
Page 2 printed with no issue.
Page 3 had overlay of two passes of printing on same page.
Page 4 had overlay of three passes of printing on same page.
Page 5 printed with no issues.
Page 6 printed blank.
Page 7 printed blank.
Troubleshooting shows it's not user, computer, printer, or printer driver related. Does same thing regardless of user, workstation, printer or printer driver.
Any idea what might be causing this problem in the code?
